Emergency Contact Cards

Hello STARS Families,

Your student was sent home with an emergency contact information card. Please take a few minutes and verify the information and make any changes that might be needed. Please list all authorized people in the Emergency Contact and Authorization for Pick Up section. Only people listed there will be allowed to pick up your student, unless special arrangements are made through the front office. Once all information is recorded please sign at the bottom where indicated and return to school.
